Patient Referral
The medical staff members at the Cosmopolitan International Diabetes Center are
committed to developing a continuing relationship with patients' current physicians
to ensure the best care throughout patients' lives.
Physicians who would like to make an appointment for a patient may call Referral Services at 1-800-877-7197, or they may call the clinic directly at (573) 882-3818 and speak to one of our staff physicians. To schedule a referral appointment, the following patient information will be required:
- full name
- date of birth
- Social Security number
- address and phone numbers for day and evening
- whether the patient has previously been treated at University Hospital and Clinics, either as an inpatient or outpatient.
